Real heroes:
how the physician and physician assistant
from SibMedCenter in Magadan saved people

In September two healthcare practitioners of SibMedCenter working on the project Pole-Magadan demonstrated special kind of courage and professional skills and showed by example what it means to render assistance in the hour of need.

These persons are physician Aleksey Osipov and physician assistant Yevgeny Erdygin who are working in the medical aid station of the of SibMedCenter (project Pole-Magadan). We tell the story from the accounts of the eyewitnesses.
Escorting the patient with severe trauma across the country
Aleksey Osipov, physician of the medical aid station, surgeon, dedicated his professional career to the life support activities. He started his work in the community hospital, then worked in the emergency call service and simultaneously in the surgical department. A year ago, he decided to deal with the healthcare at the remote sites and began his work in SibMedCenter.

In the middle of September one worker fell from the height of 7 meters onto the hard surface and received catatrauma. After examination closed craniocerebral injury, closed olecranon fracture, composite multifragment fracture and fracture of pelvic bones were found out.

The injured was given assistance and then brought to the Central District Hospital and later to the Magadan Regional Hospital. In the course of transportation the patient state was stable. The patient was given additional support- infusion therapy- and his vital signs were monitored.

In the hospital the injured was operated and antishock measures were conducted. Physician Osipov made observations and studied the clinical record. During a month the patient was in the supine position in the hospital. After that the patient needed rehabilitation- keeping the supine position and recovery of the functions of the extremities. Osipov spoke with the colleagues and they decided to bring the patient home.

The personnel of the Regional Hospital prepared the patient for transportation and the Disaster Medicine Service rendered assistance in transfer to the airport. Aleksey Osipov together with the patient took the flight Magadan- Moscow. The patient was on the stretcher which were placed fixed in the passenger compartment. During the flight the physician monitored the patient vital signs.

Aleksey Osipov said: "After such severe trauma a patient may respond to the flight in different ways, especially because the flight was quite long- 7 hours, the height of the flight was about ten kilometers, and this is quite high. However, when transporting emergency patients by air, the flying and takeoff are not as dangerous as landing".

After landing in Moscow the patient was evacuated to the Sheremetyevo medical station, he was observed within 24 hours and after that the patient was taken on board for transfer from Moscow to Nizhnekamsk (flight duration 1 hour 40 minutes). The flight was quite unproblematic. An ambulance was waiting for the patient at the airport, it carried the patient home, where everything was ready for him. The physician gave recommendations relating to the care, nutrition, and the patient was left in the hands of the district doctor and traumatologist. Up till now the patient state is monitored by the doctors and Aleksey Osipov calls on the phone the patient's brother and gives consultations when required.

Aleksey Osipov also added: "If the patient state had not been stable, I would have refused from his transportation. This is not a simple situation. If the patient state aggravates while in the air, naturally, nobody is going to land the airplane, this can be done only in the most extreme cases. That is why I had all necessary medicine and for life support gear with me. The healthcare service of the Pole-Magadan was engaged in organization of the transportation logistics and everything was done in a right way. The airport services in Magadan and Moscow and stewards were really good. Everything was just in time and faultless".
Life support in the sever car accident
Physician assistant of the medical aid station Yevgeny Erdygin many years worked in the emergency call service and is working in SibMedCenter for 4 years already.

Yevgeny together with his colleagues returned from Magadan to the mine deposit. 30 kilometers from the settlement Usty-Omchug there happened a car accident – Lada Niva crashed into truck KAMAZ.

According to the eyewitnesses the car accident had happened 10 minutes prior to the moment when Yevgeny with his colleagues appeared at the place of accident. There were two injured in the Lada Niva car, they were squeezed at the front seat by the crashed vehicle. It was impossible to take them out, their legs were broken and blocked up. Those persons who were there tried to help to the injured, however, they could not call an ambulance –phone coverage was absent.

According to Yevgeny one of the injured was conscious and groaned with pain, the other injured was in sopor and in the life-threatening condition. Pressure of the second injured was lower than 90, peripheral pulse was not palpated, however, carotid artery was felt. Waiting for the rescuers Yevgeny conducted emergency infusion therapy.

Yevgeny had with him satellite phone and while he was rendering aid, his colleagues called the ambulance and MES. While waiting for the rescue service Yevgeny kept the injured alive.

When the rescuers arrived, they cut the vehicle by the hydraulic apparatus and took out the injured. When the ambulance came Yevgeny helped to take the injured into the car, to fix the cervical collar and to rig up a splint. There was no need for the physician assistant to help with further transportation.

Yevgeny commented on the situation: "The people were lucky, that a medical person appeared to be nearby with first aid kit and satellite phone. The road was not good- it was unpaved. The maximum speed was 60 kilometers per hour. The rescuers arrived in 40 minutes, one hour passed from the moment of the car accident. This is quite a long time".

Later Yevgeny made a phone call to the hospital to make sure that the injured persons were delivered there. In the hospital its was found out that the injured had fractures of upper and lower extremities and traumas.

Yevgeny added: "In SibMedCenter physicians pass life support training courses-ITLS and ACLS. They are very helpful in further work. One can remember the procedures to be followed and to finish them with automatism. So, in a real situation you can fulfill the algorithm without hesitation".
